Keeping aside my name for a moment (:P), I'd like to thank you for the painstaking detail you went into with this one, very intricately outlining every food or habit that contributed to the problem of GERD and what go-around you used for it.
I have experienced many of the symptoms you've outlined in this article; the stomach rumbling, the excess gas, that fiery burning sensation in the throat and that nasty vile liquid sent up from the stomach to be expectorated.
Although I don't have a chronic cough from acid reflux, I do know how to control it when it strikes (having identified my personal trigger foods) from my experimentation with food and lifestyle just like you have. I have personally felt how changes in sleep routine, stress levels, exercise, and work schedule can alter how your body responds to unhealthy food and excess caffeine, and vice versa.
Now that I'm a pilot of my own aircraft, I know exactly which knob to tweak at what time and for how long. This has dramatically helped in bringing down the frequency of the most violent episodes of GERD that I've had in the last few years, and I've never had an intense episode ever since I learnt how to play with the buttons.
